What is an Anal Speculum
Speculums are often associated with vaginal examinations, although the same type of instrument may also be used for anal examination. An anal speculum is a medical tool that is designed to enable a doctor to look into the interior of certain body parts, specifically the vagina and the anus. The use of the anal speculum has a long history in anal surgery and has been dated back as far as the Ancient Greeks. Originally a speculum was made only of steel because it was easy to sterilise, nowadays with modern medical advances many of the specula used in hospital emergency departments are made of a particular type of plastic. Specula come in different shapes depending on their purpose.
Specula have tubes or blades that are meant to allow the doctor to take a look at the area where the problem has occurred. An anal speculum will hold the anus open, which means the doctor can then insert another instrument if he or she needs to take a biopsy or undertake some other procedure. There are several different types of speculum and one that is designed to deal with anal problems and surgery, sometimes referred to as an anoscope, this particular speculum is shaped like a tube with a bullet shaped piece of metal that is inserted into it.
Anal Examination
When a doctor places the speculum inside the opening of the anus it dilates that part of the body to the same width as the speculum tube. The bullet shaped insert is taken out once the speculum is in the anus, allowing the doctor to see into the anal cavity. The head of the speculum is usually covered in a jelly that dissolves in water. The blades are closed and once it is fully inserted the doctor can then open the blades. Once the blades of the tube are opened fully, they can be held in position with a locking screw, this keeps the doctor's hands free for any other procedures. The tube with the bullet shaped insert is the oldest type of speculum still being used.
Types of Anal Speculum
There are different types of anal speculum, some have one blade, some two and some three. The types of anal speculum are as follows:
- Tubular Shape - Fergusson and glass speculum
- One blade - the Czemy
- Two blades - there are eight different types of two bladed or bivalved speculum
Reasons for Using an Anal Speculum
The anus is actually the opening to the rectum, and it is usually this area that the doctor wishes to investigate. A doctor might decide to use an anal speculum if he has reason to believe that one of the following conditions are present:
- Hemorrhoids or piles
- Polyps - benign lumps
- Inflammation
- Tumours
- Intestinal bleeding
- Anal fissures
- Removing piles
- Taking a biopsy - a piece of tissue, usually to determine whether or not cancer is present.
